Dental Insurance Company Denying Authorization for Covered Plan Services.?

Anthem Blue Cross Dental Net I was referred to an oral surgeon by my primary care dentist since I have to have 6 teeth and 2 residual root tips extracted in advance of getting a full upper denture. All the extractions including the root tips have to be done during one visit, which necessitates the use of IV sedation. My general dentist cannot perform any of this work. Anthem Blue Cross Dental Net denied the original pre-authorization request, and subsequent appeal stating that my general dentist can perform the services being referred. Which is absolutely not the case. I even spoke with a customer service rep who indicated that the extraction of residual root tips is definitely a referable service and it clearly states that in their Dental Net Provider Contract. She has no explanation for why the appeal was denied. My primary care dentist has even provided Anthem Blue Cross Dental Net with his specific reasons for referring this work to an oral surgeon and I still can't get anyone at Anthem Blue Cross to re-evaluate the original denial and the appeal and authorize the necessary work. I have filed a complaint with the California Department of Insurance, although I don't hold out much hope for any results from that effort. HELP... what are my options? Who can I contact? Thanks

Wow dont ask me how, but I lucked up and found some RIDICULOUSLY cheap insurance. I have been looking for about a day and a half. Every last MAJOR insurance company you can think of. For some reason everyone on here kept saying check out Progessive. They were one of the MOST expensive. If you have a SUPER sport bike. Meaning either the Kawaski 12 or 14 or the Suzuki 1300 Busa, everyone is quoting you between $6,000 to $8,000 for a year. Thats like $400-$500 a month. That is the most ridiculous thing I have ever seen in my life. But then my salesman told me to call State Farm. He said trust him, that he used to race sportsbikes and always insured through state farm. So I call them and get a quote for $767 for the year. The same Bike that EVERYONE else quoted at $1000. Honestly I could not belive the lady. I do not understand how the price is that different. I have a car that I have a loan on that has been wrecked. Since the bank owns the car, I have full coverage & insurance is writing it off. They are going to cut a check to the bank for the balance due on the car. I have so far paid this loan on time; it has been rebuilding my credit. I had a claim against a guys insurance company for damage he caused. They came out today and did an evaluation and now the insurance estimator is saying that the vehicle needs to be totaled and that I will be offered some low ball figure because my car has prior damage which was only cosmetic and minor nothing structural at all. This afternoon, I was at a gas station and I backed my 2003 Pontiac mini van into a concrete barrier very hard. The left rear bumper is falling off, the hatch is dented fairly bad in the corner so it is difficult to open (that dent is about 4 inches in diameter but very deep) and the side with an automatic door has two shallow dents about a foot across. One of my taillights chipped on the side but didn't crack or shatter. No broken glass or anything. car insurance is at rediculous rates. government says everyone has to have it , and insurance companies are making it almost impossible to afford. Insurance coverage after my car was totaled?

In 2005 I was in an accident, not my fault. My 1997car was totaled, due to a small area of body damage. I kept the car. Comparing notes with my friend, he said I should not have been paying for collision these past 4 years. When I called to tell my insurance company I bought a new car, I asked why my rates didn't go down after the accident. They said only I could have made a change. Question: what would have happened had I been in another accident that would have totaled my car a second time. Would they deny me. Are they obliged to counsel customers about keeping collision insurance on an already totaled car. Do I have any recourse. Could I get a refund. (My question mark key is gone on my keyboard.)

Last week I used autotrader to search for numerous quotes and found one for around 1450. Today I passed my driving test, and went back to the quote but it had to be renewed since last week which is fair enough. The lowest value today was 1576 which I am alright with. I refreshed the page a while later to find that quote had disappeared and the cheapest was 1620, with the company for that quote (elephant) at the bottom with an unable to find a quote detail. I thought it would come back soon, so I refreshed the page and then suddenly the price increased to 2950 minimum and many of the cheaper companies were now listed as unable to find a quote. Strange, but perhaps the websites are too busy to send data. An hour on though and the price is still 2950. Did anyone else have this problem? If so when was it fixed, the same day or next day? This december I got paid on the 18th and i have left no money in there for my direct debit to come out when they took it out on 29th decemeber. It was dark and raining. I just came off the bay bridge in SF and am positive I was driving below 40 on the bridge and between 40-60 when I got off the bridge. The cop was on 80E past fremont exit. My job doesn't provide insurance because it's technically not full time. I absolutely cannot be without medical insurance because I deal with things like depression, asthma, allergies, and back and neck pain from an accident (I need an insurance with very good prescription drug coverage, chiropractic care coverage, and mental health coverage-like therapy/psychiatrists). Needless to say I can't afford to NOT have insurance. I'm also overweight, so it's very hard to find affordable insurance for myself. Since February, I have been on individual insurance with CHAND, a high risk insurance division of Blue Cross Blue Shield of North Dakota. The insurance is pretty good, but I pay $260/month, which is way out of my budget. My question is this: are there any good and inexpensive insurances out there for me? Me and a friend were taking out some car insurance quotes the other day - entering his information in he got 1700 a year on an old '95 (i think) cincequento, which frankly was hilarious. Then, all we changed was the address so that he lived at my house. It pushed the insurance up to something like 2700. Now I dont get this - he lives in a pretty rough area in my town, right near where all the boy racers live and such. I however live in a small village about 2 miles away and just outside of the town. In my area, the population is about 90% seniors and it is very quiet. I thought insurance companies were supposed to weigh the risks of location against their policy? I am just curious. My old ex GF had 3 kids before age 21. One of which ended up needed urgent care after birth and died in the hospital a few days later. Hi. On April first, my neighbor hit my dad's car in our neighborhood. Our garages are facing each other, and my dad was going backwards to get out of our garage. He was already in the street and he was ready to go in his way. Suddenly, our neighbor back out from her garage very fast and hit the driver door of my car. She didn't stay in that position, she moved and my dad did the same. The white bumper of her van had no visible damages, but my door needs to be replaced. Also, there is white paint in car in the place she hit it. We asked for her insurance, but she didn't have it in that moment. We wrote the plate number and took some quick pictures. I called my insurance to report the accident. In the night, when she returned home, we asked for her insurance, but she refused. We called the police and she agreed to give us her insurance information. In fact, she wrote the information herself. I called her insurance(Geico) the next day, and she had already made a claim, and she only said she didn't hit my car; she didn't give more details. My dad gave his version of the accident with a lot of details, but she didn't do it. Her Insurance tried to contact her, but there is no answer from her. I see her everyday, so I am sure is not responding on purpose. Now, her insurance called me and told me that because she is not responding, there is no way to confirm she was involved in the accident. therefore, they denied liability. I was shocked because they took that decision only because they cannot contact her. They didn't came to take pictures of the place or the cars. So, I tell them I was going to send the pictures I took. I did send the pictures but now, the adjuster is not in his office, and he will be back next week on Thursday. Right now, I don't know what to do, I could pay for the reparation of my car, but I don't believe that's fair. Please, if someone can guide me with this case. What can I do?

I'm currently with Progressive and have been very pleased with their service: I was in an accident in August and the entire process was very smooth and took little effort on my part. I worked with one local agent, dropped my car off at the Progressive repair center where they had my rental waiting for me, and returned the rental there when I came to pick my car up. Plus I never have to wait long to talk to someone when I call. However, I'm newly married and have found we would save a considerable amount by moving my two cars to my husbands policy with State Farm as opposed to moving his car to my policy with Progressive. Please let me know what type of experiences you've had with State Farm and if you would consider their service as excellent as I have with Progressive. Even if State Farm is cheaper, it won't be worth the savings if their service is crummy. Thanks!

I'm about to get my first car and I want to make sure I have the proper insurance. I don't want an insurance company talking me into buying more than I need, nor do I want to be under-protected. So what's most important? I thought I'd heard that liability coverage was more important than collision. Is that true? How much liability should I need? Why am I getting such high auto insurance quotes?

I'm really frustrated. I wanted to finally get auto insurance next month. But the free quotes I'm getting on insurance websites (such as Progressive, AllState and Geico) is WAY too expensive! My brothers insurances were always like $160-$180 for 6 months. But I just got a quote from AllState for over $500 for 6 months and $381 for 6 months from Progressive. That's NUTS! False Auto Insurance Claim Against A Friend?

A very close friend of mine received a call from his auto insurer yesterday regarding a 3rd party claim against him. According to the 3rd party, my friend hit their parked car (which was parked on the curb in front of their home), then parked his own car, went up to their house, rang their doorbell and exchanged insurance information. The 3rd party claims that this took place in a town that's more than 1 hour away from where we work and live. This allegedly happened 3 months ago. There are two major problems with this story: A- The alleged incident never happened. I carpool with my friend to and from work and was with him that night. B- The 3rd party has all of his insurance information. Could the insurance company have called the wrong person (i.e. gotten policy numbers mixed-up while processing the claim)? If not, how could a 3rd party have his insurance information? How common are fraudulent insurance claims (against another person)? And has anything similar happened to you or someone you know? My friend is a little worried because there is some minor damage to his rear bumper. We live in a major city and pretty much every 3rd car has nicks and scratches along the bumper from parking garage 'mysteries' and whatnot. Also, about 8 months ago, a young woman rear-ended us while we stationary, waiting to turn left at an intersection. Other than a scratch to his rear bumper caused by her license plate bolt, there was no cosmetic damage, and while we exchanged insurance information, my friend decided not to file a claim (she was very nice and didn't look as though she had a lot of money). I'd hate to think that it's her trying to commit fraud against him...

Ok. i have a direct debit set up with swinton for my car insurance every 26th of the month. This december I got paid on the 18th and i have left no money in there for my direct debit to come out when they took it out on 29th decemeber. I didnt realise this happened until friday 29th january and got a phonecall form them telling me they are cancelling my car insurance. I was angry as I had no letters to confirm this and no phonecalls. My phone is broken and I had changed my address (which i have given them about 10 times) they said there was nothing i could do and cancelled my insurance. They have now told me that because i havent paid that its been sent to the debt collectors and is a criminal offence!!! My argument is they did not change my address even after I had told them time after time. Is there anything I can do, or if anyone has any advice what I do now. I have set up a new insurance now but its 8 more than my last one!!
